What else is connected?
Isolate the problem, disconnect everything and start checking with the Pi only. Then connect your other components one by one, always checking what happens.
Quite frequent this phenomemena happens from devices back-powering the Pi (or bend pins on the fan connector so: did you disconnect and reconnected it)?
