Have you connected SMBDATA and SMBCLK to CM4? And they were floating when you you removed CM4 and soldered an USB cable? If yes, then probably the hub chip worked using internal default configuration.
In the description you say that the configuration is using the self powered options, but a label in the diagram states that it is externally powered. If the intention is to use internal default configuration, connect pins 13, 14 and 17 (47k resistors) as per the datasheet and do not connect 13 or 14 to CM4. If the intention is to configure the chip externally, then that is probably the issue (which means that soldering an USB cable is not telling much about the issue).
Disclaimer: this might not be the issue: I just took a quick look and 2k2 resistor as well as floating SMBD and SMBC pins got my attention...
